Medication Safety Committee

MSC is responsible to advocate and share good practices with the ultimate goal to minimise the risk of medication errors.

Terms of Reference

1. To advocate culture of medication safety in HA;
2. To propose corporate strategies, formulate guidelines and provide training and advice on medication safety to DMC;
3. To ensure sustained and effective implementation of medication safety related policies / guidelines in HA;
4. To promote culture of reporting, analysing and sharing of medication incidents / near misses in HA; and
5. To report to DMC on issues related to medication safety in HA.

Composition

1. Chairperson
   1. Senior Management Executive appointed by DMC
2. Members
   1. Chief Pharmacist or delegate;
   2. Cluster physician representatives nominated by respective CCEs from all clusters;
   3. Cluster pharmacist representatives nominated by respective CCEs from all clusters;
   4. Cluster nurse representatives nominated by respective CCEs from all clusters;
   5. Chief Manager (Patient Safety & Risk Management) of Quality and Safety Division, HA Head Office
   6. Representative of Nursing Services Department, HA Head Office; and
   7. Representative of Information Technology and Health Informatics, HA Head Office.

   • Pharmacist of CPO

Individual cluster representatives, nominated by Cluster Chief Executives, are mostly involved in Quality & Safety or Medication Safety improvement exercises in their own clusters.

With representatives from both hospitals and HA Head Office, MSC aims to facilitate exchange of ideas to improve medication safety, discussion of medication safety-related issues and implementation of guidelines across HA institutions.
