In order to cater for the needs of different patients, a wide range of patient services are rendered on either outpatient or ambulatory basis. Services include Family Medicine Clinic, different Specialty Clinics, Diabetes Centre, Rehabilitation Day Hospital, Integrated Community Rehabilitation Centre and Cardiac Rehabilitation & Resource Centre.

 

 



Family Medicine Clinic


Services include

Provide basic consultation services to the public

Provide extended health care services to patients with chronic diseases

Provide consultation services to the hospital staff

Dressing & Injection Service Registration Hours

Telephone Appointment Service

To facilitate patient's access to the General Out-Patient Department, a 24-hour Telephone Appointment Booking Service is implemented for patients with episodic illnesses to make appointments for consultation and treatment by telephone. Clients can dial the designated telephone number and follow the system instructions to make an appointment of available session in the next 24 hours. Target Clients: Applicable to registered patients of the clinic. New cases should bring along the identity document and register in person at the registration counter before using the telephone booking service.

Booking Methods for New Case patients


New case patients can reserve an appointment in the specialist out-patient clinics by:

In-person
Patient should present the valid referral letter* from a registered doctor in Hong Kong, patient's identity document and address record and make appointment at the Registration Counter of the Specialist Out-Patient Department (Medicine & Rehabilitation) at G/F, Main Block during office hoursu#.

Fax booking
Patient should fax the valid referral letter* from a registered doctor in Hong Kong , a copy of his/her identity card and address record with the contact phone number, fax number and correspondence address stated to our Specialist Out-Patient Department (Medicine & Rehabilitation). The referral would be screened by our doctors upon which confirmation of the appointment date/time will be given to the respective patient by phone or fax.

* The referral letter is valid within three months from the date of issue. Appointment given by nurse is based on New Case Classification Guidelines. Photocopies of referrals would be given to doctor afterwards to confirm priority of condition and /or re-schedule appointment.
For new appointment defaulted cases, a valid new referral letter is required for re-booking. Change of appointment should be done before the appointment date
