  • Booking Arrangement of New Case
  1. For arrangement of new case appointment, patient or relative should bring the following documents:

    1. Valid referral letter¡]Please verify patient¡¦s Chinese & English name on the referral letter with the HKID or travel document¡^and
    2. Patient Identity document copy

  4. The valid period of referral letters:
    Patients are advised to make appointment as soon as possible with the valid referral letter. If the effective period of referral letter is over, patients have to get a new valid referral from their referring doctor. Please note that, the validity of referral letter will be six months only from the issued date for all specialties.


  5. Booking by Fax:
    If patient cannot make appointment in person, patient can fax the following information to our centre for booking :

    1. Valid referral letter
    2. Patient Identity document (HKID)
    3. Contact address & telephone number

  6. Points to Note for New Case Patient Attending Consultation
    1. New case patient should bring along with the original valid referral letter

    2. Please attend on the specified date and time printed on the appointment slip accordingly

    3. Patients who do not turn up on the appointment day can reschedule another appointment within the effective period of referral letters in person or by fax

    4. Patients should bring along the valid original identity documents, such as Hong Kong Identity Card, Hong Kong Birth Certificate. If patient is a Non-entitled person, please produce valid travel document

    5. Patient can contact our Centre staff for changing telephone number, address and personal particular

    6. In case of serious discomfort before the appointment date, please immediately seek treatment at Accident & Emergency Department or your family doctor

    7. The charges for Non-entitled person (those who fail to produce identity document of Hong Kong resident) is according to the Hospital Authority announcement

    8. If patients have private X-ray films and other physical examination results etc, please bring along for consultation
