| Medical Report Application | Application form for medical
report can be obtained from Enquiry Office at G/F, Central Stack or
download from the link below. The charge for each medical report is
from HK$695.00 to HK$2,780.00, depending
on the type and professional services required. Applicant may pay
at the Hospital's Shroff Office which is located at G/F, Central Stack.
The completed application form and official receipt should be returned
