Psychogeriatric Services
Psychogeriatric Team provides comprehensive psychogeriatric assessment, treatment and rehabilitation programme for psychogeriatric clients over 65.
From October 2002, the Elderly Suicide Prevention Team provides psychogeriatric service to prevent elderly suicide in partnership with relevant community organizations, GPs and related health care professionals. (see details below)

Community Services |

Outreaching and community services are provided to subvented elderly centres in Tsuen Wan, Kwai Tsing, Shamshuipo and Tung Chung districts. Through collaboration with the Social Welfare Department and some Non-Government Organizations, the team provides psychogeriatric nursing assessment service, community dementia care programme, walk-in clinic, and supports the Enhanced Home and Community Service provided by the NGOs. |

Out-patient services are provided at 3/F of Ha Kwai Chung Polyclinc and 8/F Block L of Kwai Chung Hospital. |

Day Hospital Services |
|
Different day hospital services are provided for psychogeriatric patients in Kwai Chung Hospital : Psychogeriatric Day Hospital and Psychogeriatric Day Care Unit for Dementia at 8/F Block G; and Psychogeriatric Ambulatory Care Centre at 8/F Block L. |

In-patient Services |

Kwai Chung Hospital provides short-stay inpatient services for psychogeriatric patients, which locates at 8/F of Block M. It provides comprehensive assessment and management for elderly patients with acute psychiatric problems. |

They include Memory Clinic, Mild Cognitive Impairment Clinic, Mood Disorder Clinic, psychogeriatric out-reaching nursing service to private old age homes, Community Psychogeriatric Nursing Service, Dementia Resources Centre, and Carer Support Centre, etc. |

Kwai Chung Hospital
Elderly Suicide Prevention Team
Scope of Service
The Elderly Suicide Prevention Team provides psychogeriatric service to prevent elderly suicide in partnership with relevant community organizations, GPs and related health care professionals.
The team would provide training to local GPs, related professionals and organize community health education for the local public in handling depressed elderly and suicide prevention.
Target Client Group
All elderly clients aged 65 or above living in Sham Shui Po, Kwai Chung, Tsing Yi and Tsuen Wan Districts referred by related referrers.
Mode of Service
The service operates as a two-tier model. The first tier is mainly a referral network with NGOs, DSW, volunteers, GPs, GOPD and other general hospitals within the Mega-cluster. The second tier consists of Fast Track Clinic, Home visits supplemented with telecheck and elderly suicide early intervention for assessment and management of suicidal elderly.
Consultation at Fast Track Clinic would be arranged within 7 working days.
Nurses would make use of home visits supplemented with telecheck process for monitoring the clientsˇ¦ progress and condition.
The team regarded those cases with serious risk of suicide as ˇ§crisis casesˇ¨. Nurses of the Team would visit these clients within 7 working days. The nurses would consider the need to refer the case to CPT, KCH for urgent consultation. After assessment, the CPT could then refer back the case to our team for follow up.
If the referrer considers the client needed immediate intervention, they are advised to refer the case to nearby A & E Dept. or CPT, KCH for urgent consultation.
For DSW, NGOs and Hotlines, please refer to Appendix II for the flow-chart on the referral system for social worker staff. The standard screening form for referral is attached at Appendix III.
Guideline for GP and other medical referral to Fast Track Clinic as recommended by Steering Committee on Elderly Suicide Prevention Programme is as follows:
When the client:
- Expresses suicidal thought or
- Has plan for suicide or
- Likely to attempt suicide (GP's clinical judgement) or
- Previous attempt of suicide or
- Moderate to severe depression at risk of suicide.
For General Hospital within Kowloon West Cluster, doctors can use the Referral Form (Appendix IV) or memo form.
