Referral for Different Types of Palliative Care Services
Palliative care services include in-patient, consultative, outpatient, home care, day care, psychosocial and bereavement services. Referral criteria have been set for different types of palliative care services and suitable cases could be referred to different palliative care units according to their referral network. Some palliative care units accept referrals from all hospitals, including private hospitals/clinics, while some other palliative care units mainly receive referrals from own hospitals and other HA hospitals within the cluster. Standard referral form for palliative care will need to be filled for making referrals for palliative care.
